About the Role
As an Advanced Practice Provider with HarmonyCares, you will have the opportunity to:
- Manage a small panel of elderly patients
- Dedicate 30-60 minutes to each patient, providing personalized and attentive care

Qualifications
- Nurse Practitioner (IL) license and (AANP or ANCC) Board Certification in good standing
- Experience in geriatric care
- Excellent communication and interpersonal skills
- Compassionate and patient-centered approach
