Quality Coordinator, RN
Job Description
Under the supervision of the Chief Quality Officer (System), the Quality Coordinator plays a vital role in our healthcare delivery system. Primary areas of accountability may include:
* Creation and execution of hospital wide "Conversation Ready" Initiative
* Point person for any ACP/EOL needs on patients without Palliative Care on board
* Supportive role to all clinicians to ensure thorough ACP
* Supportive role to all clinicians to provide the highest level of care to patients nearing EOL.
* Ongoing training/involvement for designated "Conversation Champions"
* Creation of educational and interactive resources for patients and their families to assist with EOL decision making
* Identification of triggers to start palliative and hospice consultations earlier in the hospitalization
* Ongoing clinician palliative concepts education
* Conversation Ready educational classes
* Skills Day Conversation Ready education
* Development of Cerner documentation to capture ACP
* Development of note templates to capture ACP conversations ? Continued process improvement/growth of the initiative
* Data collection on impact of the initiative
* Assist with goals of GIP hospice initiative
* Nurse education
* Nurse GIP conversion protocol
* Hospice liaison
* Supportive role to bedside nurses to provide highest level of care to patients under to hospice care
