    Job Details
  • Transitional Care – LTC/SNF. Shift: Nights. Number of Openings: Multiple. Weekly Guaranteed Hours: 48 GH preferred, 36 Acceptable. Weekend Hours: Yes, Every other weekend. No Call Requirements. Years of Experience: 1. May be floated based on skillset.
    Job Requirements
  • Must have recent Acute Care Experience, Charting / EMR Experience. Education Modules: Healthstream Modules.
    Schedule Information
  • Nights shift. 48 hours per week preferred, 36 hours acceptable. Every other weekend work required.
    Unit Specific Information
  • Patient Care: Mix of Medical and Surgical, Medically ready for Discharge. Personality Fit: patient-centered care, teamwork, excellent communicator, punctual, reliable.
    Additional Details
  • Extension for position available.
