Certification Details BLS NRP AWONN/Fetal Heart Monitoring License/Certificate Verification Job Requirements Must include an image the front of card as well as an image of the back of the card. For any position requiring a license or certification, attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days. Additional Details 9 Panel completed within 30 days of start date, preferred within 14 days of start. Must be completed and resulted before start with MRO signature. Must include the following: Cocaine, Amphetamines, Opiates, 6- Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, and Methadone. (NO THC). All engaged contractors must have signed forms on file in Trinity VMS, including Trinity Code of Conduct Form, Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement, Trinity Integrity and Audit Services Form, Trinity I9, Trinity MVR (Motor Vehicle Report). Proof of vaccine, declination or exemption(with agency letterhead). Agency will provide Trinity Skills Checklist for clinical contractors.
