  • American Heart Association certification is required for all direct patient care personnel.
  • American Heart Association certification is required for RNs in CCU, ED and PACU, MICU, SICU.

  • For any position requiring a license or certification, attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days.
  • IA Only
  • Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check *required* for all engaged contractors.
  • Attach scanned report to contractor's record in Trinity VMS. By Law this must be done within 30 days prior to start.
