We are seeking experienced Day Center CNAs to join our team!

Job Summary

We are actively recruiting qualified, Certified Nursing Assistants to work with our Day Center team. This position requires a dynamic individual who enjoys working in an environment that encourages developing relationships with our participants. Our team-focused approach to care is the cornerstone of our organization. The Certified Nursing Assistant performs participant care activities and other tasks as assigned to assist the primary care, therapy, and adult day health staff. Works under the direct supervision of the Day Center Supervisor. This is a part-time opportunity. We are seeking candidates who can work between 10:00 AM - 2:00 PM EST, Monday-Friday.

Specifications
Education: High School Diploma or equivalent

Experience: Six (6) months of clinical experience as a personal care aide, nursing assistant, or emergency medical technician

Number/Type of Employees Supervised: None

Licensure, Registry, or Certification Required: Nursing Assistant certification in the State of Michigan; Valid MI Driver's License with good driving record; CPR certification

Special Training: Only acts within the scope of practice. Meet a standardized set of orientation and/or individual competencies for the specific position description established by PACE of Southwest Michigan, and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Specific Requirements and Considerations: May be subject to exposure and handling of infectious waste, diseases, conditions or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or Hepatitis B.

Requirements:

Key Responsibilities

Communicates effectively with participants, participant's family member(s), and team members.

Works effectively in a team setting.

Provides basic ADL nursing care to participants in the PACE Day Center to include feeding, bathing, and toileting.

Clean facility equipment and main facility conditions.

Report the participant's medical status and disposition to the Day Center Supervisor.

Demonstrates active participation in Quality Improvement process.

Participates in the process of teaching and learning with others.

Conducts SBAR's as needed and reports findings to Clinic Lead.

Abides by all established PACE of Southwest Michigan policies, rules, and regulations.

Other duties as assigned within job scope.

Skills and Knowledge

Excellent attention to detail and organizational skills.

Proficient verbal and written communication skills.

Ability to work a flexible work schedule.

Knowledge of physical, mental, and social needs of frail older adults.

Effective skills in physical assessment and chronic disease management for frail older adults.

Able to work within the Interdisciplinary Team (IDT) setting.

Able to utilize basic computer skills in the workplace.

Specific vision abilities required by this job include close vision, distance vision, and the ability to focus.
