Location: Maui, Hawaii; East side of the Island Start Date: ASAP; as soon as credentialed; preferably by Nov 1 End Date: Ongoing; looking for a 3-6 month commitment Schedule: 3 days/week in Clinic (Thursday through Saturday) + Call 3-4 nights per week EMR: Athena Patient Load: 15-18 patients per clinic shift; Call back is 50% Requirements:

• Active, unrestricted HI License & DEA
• Board Certified in Family Medicine
• Clean Background/DEA/Malpractice/NPDB
• Certifications: HI State Narcotics Registration, BLS, ACLS, & PALS
• Covid vaccinations + booster; 2-step TB Test
• Must submit updated CV with application, as well as HI License, DEA, Board Certification, NPDB, Medical Education Diplomas
