  • Dates of Coverage: Beginning 1/1/25
  • What Percent of the Practice is Adult: 100%
  • What Percent of the Practice is Outpatient: 100
  • Average Number of Office/Clinic Patients Per Locum Per Day: 6-7
  • Will Other Neurologists be Present During Locum's Assignment? No
  • How Many Physicians in Your Group? 1
  • Documentation System/EMR: EPIC
    Facility will reimburse for the Following Costs: Hotel, Rental Car
Submission Requirements:
  • Board Certified/Board Eligible: Palliative Care, Hospice, Neurology, Physiatry, even Internal Med or Critical care with experience in ALS
  • NY License
  • BLS/ACLS
