Role Overview

The hospice aide is responsible for providing personal care under the direction of a registered nurse.

Responsibilities
n
n- Document care provided according to organization policy.
n- Report any deviations from the Plan of Care to the Case Manager
n- Use Durable Medical Equipment safely.
n- Take vital signs when on Hospice Aide Plan of Care.
n- Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care any by active participation in team and staff conferences.
n- Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager.
n- Assist with ambulation, transfer, range of motion if documented on the Hospice Aide Plan of Care.
n- Provide respite for patients family/caregiver when on-site, as appropriate.
n- Participate in QAPI activities.
n- Participate in IDG meeting, as appropriate.

Qualifications
n
n- Meet training requirements by the state of Utah.
n- One year experience as a CNA, hospice aide or home health aide.
n- At least 18 years old.
n- Must be a licensed driver with an insured automobile in good working order.
n- The ability to communicate well, both verbally and in writing.
n- Ability to work as a team and also independently
