It involves direct, written, and telephonic contacts with clients and their service providers to improve outcomes and to ensure access to care and needed supports.DESCRIPTION OF DUTIES:Initiates Care Coordination Plans and ensures accurate documentation of all data collected; maintains comprehensive electronic client files, which include release of information, assessments and other medical documents acquired on behalf of the client.Serves as the contact point, advocate and informational resource for clients, treatment team, family/caregivers, and community resources.Facilitates access to appropriate treatment providers through inquiry, referral, coordination and timely follow-up. Increases continuity of care through managing relationships and communicating information and/or facilitating delivery of documentation to all providers especially during transitions in care. Utilizes person centered approaches including Motivational Interviewing when interacting with clients.Builds and maintain positive professional relationships with community partners, clients, and co-workers, adhering to the code of ethics and agency policies.Completes all assigned continuing education requirements Performs other duties as assigned by Supervisor.QUALIFICATIONS:Medical Assistant certification and/or experience preferred.Proven work experience in a medical office setting preferred. Previous experience providing case management or care coordination or working with clients with substance use disorders.High school diploma or GED required. Relevant education, training, and expertise to meet the needs of the client and goals of the CCBHC.Maintain a valid KY Driver's license and automobile insurance coverage. SKILLS AND KNOWLEDGE REQUIREDAbility to work with persons in a manner that shows respect for cultural differences is needed.Must possess good oral, and written communication skills.Must be highly organized.
