Nurse Manager
POSITION SUMMARY: As a key member of the Media Campus nursing leadership team, the Nurse Manager provides and supports other nursing team members in providing normalized, empathetic, therapeutic, and preventative health care to individuals with intellectual disabilities. The Nurse Manager oversees the functions associated with managing the nursing needs of individuals in residence(s), care coordination, and the delivery of nursing services while ensuring that both regulatory and clinical practice standards are met.
DUTIES AND RESPONSIBILITIES:
* Ensures that ongoing clinical assessment, coordination, and interdisciplinary collaboration necessary to meet client health care needs are performed by nursing staff assigned to the unit/s they manage
* Ensures that appropriate health care assessments are completed as directed by the interdisciplinary treatment team, policy and procedures, and regulatory requirements through tasks including but not limited to quarterly and annual assessments or during initial assessments at the time of admission to the unit
* Ensures that client assessments and reports are accurate and complete
* Presents data collected through direct observation, interview, clinical assessment, and records and presents review to the physician and larger interdisciplinary team
* Obtains Certified Investigator (CI) designation and conducts investigations of incidents as necessary, e.g. death investigations, medication errors, etc.
* Sets health care priorities, including but not limited to medication administration, vital sign monitoring and wound care in accordance with professional judgment
* Supervises the nursing staff, assigned to the residences they are managing, in the implementation of the nursing process by providing assistance/support/oversight in identifying alternate interventions to promote the general health and wellness of the client
* Provides supervision of assigned staff in the areas including, but not limited to, recruitment, selection, leadership, coaching, training, performance management, promotion, discipline, and termination
* Completes the Annual Health Summary for the person's served in the programs they are managing, and presents to the larger interdisciplinary team ensuring that all health care needs have been addressed
* Works closely with family members/legal guardians/health care decision-makers for the individuals supported in the residences they are managing to identify and communicate client health care needs and actions being taken to address them
* Serves as a resource person for questions related to health and wellness, including but not limited to vital sign values, infection treatments and illness/wound status
* Ensures appropriate follow-up and documentation of all recommendations for specialized services/consultations has been completed
* Reviews recommendations of consulting pharmacist and liaisons with physicians and care providers for necessary review and follow-up activities, including but not limited to medication change, medication administration times, and alteration of medication dosages
* Monitors physician's orders, Medication Administration Records (MARs), and relevant documentation related to compliance with medication administration to ensure that medications are properly and safely administered to reduce the risk of medication errors and injuries
* Reviews critical incidents related to the health and safety needs of the consumers and ensures that there is accurate, detailed, and timely follow-up and documentation of such via daily progress notes and communication, whether in person, via phone or through email, with the Nursing Supervisor for addition to the Nursing Supervisor Shift Report
* Liaisons with external pharmacies, durable medical equipment (DME) providers, vendors, specialists, community-based providers, and insurers to ensure that client-specific needs are met in a clinically prudent and fiscally-sound manner
* Provides clinical leadership, direction, and support to the extended care team to ensure that consumer health and safety needs are adequately addressed
* Assists in accessing community-based services and care providers, especially in emergency situations
* Utilizes quality improvement strategies to address the following:
* Clinical performance of nursing staff through monthly audits of nursing documentation and care delivery
* Use of best practice guidelines in promoting health maintenance and disease prevention strategies
* Compliance with regulatory standards as they relate to specific program areas, such as licensing regulations and current policy
* Access to high quality care consistent with consumer specific needs
* Health care utilization data
* Assigns specific quality improvement activities/functions, such as medical record reviews or staff observations, to nursing staff as necessary
* Develops data management reports as needed to assess and improve quality of services provided
* In collaboration with the larger interdisciplinary treatment team, monitors, develops, and maintains a safe and therapeutic living environment, including, but not limited to, monitoring for potential threats to health and safety and take appropriate actions to eliminate such threats
* Performs cardiopulmonary resuscitation (CPR), crisis intervention, and medication administration using agency-trained protocols
* Ensures that all program based medical and nursing equipment is clean and in good working order, arranging replacement as needed
* Maintains up-to-date inventory of nutritional supplements, medical equipment, and medical/dietary supplies
* Provides round the clock on-call clinical support to staff assigned to their residence, via telephone communication with the shift supervisors, in managing emergent healthcare issues during off hours
* Provides weekend (Friday at 4:00pm to Monday at 8:00am) and holiday on-call coverage, with a rotation equal to the number of Nurse Managers employed at the Media Campus
* Ensures that other Nurse Manager coverage is identified and communicated to the Media Campus Director of Nursing, Nursing Scheduler, and Medical Assistant/Office Manager, during periods of vacation and days off (other than Holidays and Weekends)
* Provides nursing and program staff in-service training on health-related topics, medical procedures, and infection control as needed
* Participates in ongoing educational opportunities designed specifically for the consumer population served
* Attends both professional and organizational meetings necessary to keep abreast of changes in nursing practice
* Works with, understands, and complies with terms and conditions of collective bargaining agreements (CBA) within a unionized environment with tasks including but not limited to adherence to scheduling procedures, processing transfers and promotions, administering disciplinary actions, adjudicating grievances, where appropriate, etc.
* Performs other duties as assigned
EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:
* Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) to practice Nursing in the Commonwealth of Pennsylvania
* Bachelor of Science in Nursing preferred
* Four (4) years general nursing experience
* Three (3) years in the field of Intellectual/Developmental Disabilities
* Must possess strong clinical, organizational, and medical triage skills
* Two (2) years nursing supervisory experience
* Leadership/management experience working with interdisciplinary care teams preferred
* Previous experience working within and managing a unionized workforce preferred
* Non-profit human service experience highly preferred
* Demonstrated ability to work effectively as part of a team
* Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
* Demonstrated strong attention to detail
* Demonstrated strong time management and organizational skills
* Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
* Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
* Must be able to perform CPR and crisis intervention using agency-trained protocols
* Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
* Familiarity and comfort using electronic health records
* Demonstrated intermediate level of experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint
* Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
