Care Coordinator - Substance Use Recovery Focus

About the Role:

This role focuses on supporting clients in various stages of recovery from substance use. As a Care Coordinator, you will play a vital role in guiding clients toward managing their co-occurring needs and overall well-being. You will work as part of a multidisciplinary team at AWS, providing high-quality, holistic care to clients residing in substance use treatment programs. In addition, most of the clients you will be working with reside in a substance-use treatment facility. Although you are an AWS employee we will need strong collaboration between this position and the residential team's assigned team member for the client. This ensures the client is getting significant support to increase their skills for independence with the appropriate support and services as they prepare to live independently in the community again. This position involves a combination of office-based work and community visits. Experience working in residential treatment settings is a plus.

You will be the point person for new admissions in the residential programs and coordinate with the AWS intake team. Initially, you'll meet with new clients weekly. Subsequently, the time dedicated to each client will depend on their individual needs. As a care coordinator, you'll play a crucial role in connecting new clients with essential services, often acting as their primary point of contact for healthcare (including primary care, medication providers, and pharmacies), mental health services, vision care, food stamp applications, housing assistance (including recovery residences), and other individualized resources.

Key Responsibilities:

  • Strong collaboration with both residential facilities, community providers, resources, and the internal AWS team.
  • Conducting psychosocial assessments
  • Developing strengths-based treatment plans that incorporate harm reduction and relapse prevention strategies
  • Coordinating care with other providers, including natural supports, BHH team members, community providers, and the residential team.
  • Promoting overall health and wellness, including education on the physical and mental effects of substance use
  • Advocating for clients' needs within the treatment system and the community
  • Providing support and guidance to clients and their families throughout the recovery journey
  • Connecting clients with community resources, including support groups and aftercare programs

Requirements:

Qualifications:

  • MHRT-C, MHRT-C provisional, or eligible for MHRT-C/p
  • Demonstrated understanding of substance use disorders and the recovery process
  • Experience working with individuals in recovery from substance use, preferably in residential settings
  • Strong communication and collaboration skills
  • Strong decision-making and independent thinking skills
  • Ability to comply with agency, state, and federal guidelines
  • Valid driver's license and reliable transportation
  • Commitment to providing client-centered, quality care

Essential Duties and Responsibilities

  • Maintain a caseload of approximately 30 clients, with an emphasis on supporting those in substance use recovery.
  • Initiate engagement within two days of intake, maintaining regular contact during Program duration.
  • Develop and implement strengths-based treatment plans incorporating harm reduction, relapse prevention, and overall wellness strategies.
