Serves as a key member of a multidisciplinary team to provide comprehensive primary care to all patients. Provides direct and indirect clinical care under the supervision of the medical group administration and providers in the care team. Must possess exceptional interpersonal, educational and training ability. Must be able to use critical thinking to prioritize needs of clinic, allowing for most efficient clinic flow, and optimal patient experience. Must put the patient first and possess a knowledge that demonstrates ability to utilize community and organizational resources effectively. Must be able to assist in the building of a well-trained, clinically competent and positive care team to support excellent quality, patient-centered care, and provider efficiency. Must possess exceptional time management skills. Willing to assist and train staff to support provider in-office procedures. Required to accommodate for and attend 80% of all scheduled meetings.
- Prepares for, attends and participates in team meetings/huddle(s) and delegates tasks to appropriate team members
- Identify clinical needs proactively by review and monitoring of monthly quality reports and communicating needs during daily huddle
- Collaborates in developing team priorities and patient goals along with other team members, administers depression screening tools
- Assess for screening needs, dependent on diagnoses
- Update health maintenance records and notifies provider of patient’s health maintenance needs
- Manage referrals
- Answer calls real time, when able
- Under the supervision of the provider, completes Annual Wellness Visit
- Actively participates in patient education that has been provided by the organization, to include goal setting, self-management, and coaching
- Assist Staffing Coordinator with coordination and maintenance of staffing schedules
- Assume responsibility for ordering supplies, equipment maintenance, overall clinic appearance / cleanliness
- Routinely evaluate clinic for Quality compliance and turns in outdated checklists on a monthly basis
- Act as a preceptor to new staff, working with Informatics and Staffing Coordinator to orient new staff to clinics
- Attend Clinic Coordinator meeting
- Ability to follow through on prescribed organizational operations.
KNOWLEDGE/EXPERIENCE: 2 years clinic experience preferred. Experience in overseeing unit operations preferred.
EDUCATION: Current Montana License as a Licensed Practical Nurse.
LICENSE/CERTIFICATION/REGISTRY: Current Montana State Board of Nursing licensure as LPN. Basic Life Support (CPR) certification and re-certification every two years.
