Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior Community Care of State (SCC). As assigned, works shifts in the SCC Day Center and in participant homes and provides care and services to participants and assists with the provision of a safe and effective day program.nRequired Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.nnMust have an active and up-to-date certification in CPR.nA minimum of one-year experience in working with the frail or elderly population.nHigh School diploma or equivalent required.nnMust have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.nnMust have a valid driver's license, proof of insurance and have means of transportation. You will be traveling to participant homes.nnPreferred:nnCurrent STNA licensenBilingual skills are a plus!nnEssentials:nnConsults each day with the Home Care Coordinator (or designee) to be updated on changes in participant status, changes in plan of care, and to confirm scheduled visits.nAssists participants in the home with the following duties:nPersonal hygiene such as skin, mouth, hair care and bathing.nAssists participants in and out of bed assisting with ambulation.nAssisting with prescribed exercise which participants and aide have been taught by appropriate personnel.nPrepares meals, including therapeutic diets, assists participants with meals.nAssist participants to the bathroom or in using commodes, bedpans, or urinals.nAssisting participants with medications which are ordinarily self-administered.nOther duties taught by a health profession for a specific participant such as: taking of vitals, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.nPerforms directed household / homemaker services.nPerforms the following duties, as assigned or needed, in the Day Center:nAssists participants and or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) in the Day Center as assigned or needed.nMonitors and assists participants with meals at feeding table.nAssists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.nPerforms and or assists with laundry, including clothes for participants.nAssists the Drivers with the loading and unloading of participants from vehicles.nnIn our 2024 Great Place to Work survey, employees said their work has a special meaning: this is not "just a job".nAt VOANS, we celebrate sharing, encouraging and embracing diversity.
