We are a local, independent, non-profit agency providing programs and services to meet the diverse needs of over 30 surrounding communities in central and northern New Hampshire. Essential Functions
  • Reasonable accommodations may be needed to enable individuals with disabilities to perform the essential functions.
    Completes at least an hour of education each month per CMS regulations and tracks it on their competency sheet.
  • Functions within the limits of LNA role and complies with all state & regulatory rules and follows the patient care plan including personnel care, housekeeping, errands, etc.
  • Informs the primary clinician / Case Manager of changes in patient condition, as appropriate and documents interdisciplinary communications.
  • Ensures that clinical competencies are completed prior to providing standard and specialized care and that competency sheet and/or patient record demonstrates.
  • Documents patient care per agency protocol and CMS regulations.
  • Collaborates with the interdisciplinary team
  • Adheres to Agency handwashing technique, infection control and bag technique.
  • Is an active participant in Case Management and agency in-services.

State License:
Current N.H. State Registration as a Licensed Nurses Assistant (LNA) and in good standing
CPR Certified within 6 months of hire
Must be able to read, write and speak English fluently
Must be able to supply a valid driver's license without restrictions
Must be able to sit for long periods of time viewing small print
Must be able to comply with PBH&HH safety and dress code policies
Must be able to meet the lifting requirements of the position
Must pass a criminal, driving record check as well as Bureau of Elderly and Adult Services
Must be able to provide eligibility to work in United States paperwork within 3 days of hire.
Must have reliable transportation and be able to supply proof of car insurance in the amount of $100,000/$300,000 coverage.
Access to operable motor vehicle and a valid NH driver's license with acceptable record of safe driving practices. Willing to drive own automobile unlimited miles and in all types of weather conditions.
Must have completed Direct Care Competencies with RN supervision before practicing independently.
Ability to pay close attention to detail and follow verbal and written instructions.
Ability to quickly adjust to change.
Ability to swiftly assimilate and interpret new knowledge and/or regulations.
Excellent interpersonal communication skills and organizational skills required.
Computer skills strongly preferred.
Work Environment
The job operates in field, entering into patient homes and locations in the community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This position also travels in their own vehicle.
Physical Demands:
This is largely a traveling role; however, some sedentary action is required. This will require the ability to lift files, open filing cabinets and bend or stand as necessary.
Travel:
Travel is expected for this position.
