About the job LNA
Responsibilities:
- Obtains and records Vital Signs and weights per policy;
- Participates in the care planning process and implements care according to care plan;
- Provide hands on patient/client care;
- Grooming of patient/client;
- Uses Standard precautions when caring for all patients. Adheres to contact and or droplet precautions as indicated;
- Feeds or assists patients with meals and provides additional nourishment and hydration per care plan;
- Emotional support for patient/client;
- Participates in quality improvement activities as requested;
- Collects and bags soiled linen and delivers to dirty linen area;
- Performs skin care treatments, as permitted by state regulation;
- Assists physician and or licensed nurse with treatments and procedures as needed;
- Collaborates and coordinates with other departments to provide timely effective care consistent with individuals needs, choices and preferences;
- Cares for personal belongings;
- Reports changes in patients condition, patient/family concerns or complaints to charge nurse and or supervisor;
- Toileting and incontinence care for patient/client.
Requirements: - Reliable transportation;
- TB shot;
- Active NH LNA license;
- Have reliable transportation and meet Agency requirements for vehicle insurance;
- Physical exam within the last year;
- Must be licensed or license eligible in VT and NH;
- Automobile insurance
