Qualifications:
n
n- Certified Nursing Assistant (CNA) license (required)
n- 12 months of caregiving experience (required) (Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice)
n- Current CPR certification (preferred)
n- Hospice experience (preferred)
n- Excellent customer service skills
n- Strong communication (written, verbal, and active listening)
n- Problem-solving skills
n- Time management skills
n- Ability to work in an electronic medical record system
n- Ability to lift 50 lbs.
n- Good understanding of body mechanics
n- Strong understanding of professional boundaries
n- Valid driver's license (required)

Responsibilities:
n
n- Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
n- Review patient care plans
n- Assist with lifting and transferring patients
n- Assist with range of motion, positioning, exercise, and ambulation
n- Assist with dining and nutrition
n- Maintain electronic medical records
n- Work under the direction of a Case Manager RN
