Job Description:

• Position Overview: Seeking a Urologist to assist with ongoing 24-hour hospital call coverage.
• Provider Pool: Providers will be added to the current pool for ongoing shifts starting in January 2025.

Job Requirements:

• Type of Care: Adults only, general urology procedures required
• Expected Patient Volume: 2-5 patients per day
• EMR System: EPIC
• Credentialing Timelines:
• Payer enrollment: 90-120 days from submission of required documentation (IN license, DEA, etc.)
• Hospital privileges: 45-60 days from complete application submission

Submission Requirements:

• Board Certification/Eligibility: Board Certified or Board Eligible within 5 years of residency
• State License: Must hold an active Indiana license
• ACLS Certification: Required
