Cabell Huntington Hospital is seeking a
full-time Colorectal Program Navigator to work alongside our
ECCC Specialist Surgical Department. The Colorectal Program Navigator will be responsible for the coordination of care for both the Colorectal Screening and Colorectal Cancer Multidisciplinary Programs. The focus of the program will be to facilitate screening and follow-up. The navigator would have the responsibility for tracking and documenting outcome data and follow-up information/outcome data; responsibility to improve outcomes. They would facilitate an atmosphere of interactive management and the development of collegial relationship across all disciplines. They will collaborate with CHH Endoscopy Services and serves as advocate to help patients and families navigate through the process of screening through acute episode of care. This position will also require the precertification of the procedure for the scheduling process. They will collaborate with physicians to ensure that patients are at the appropriate level of care with use of clinical based guidelines such as NCCN. The navigator directs the flow of patients into and out of Cabell Huntington Hospital, ECCC and University Physician & Surgeons allowing for an exceptional customer service process for our referring physicians and facilities.
- Serves as an essential link between patients and all other care providers.
- Facilitate appointments for consults with Multidisciplinary Colorectal Program Team
- Coordinate multidisciplinary planning conference; develop concise patient summaries for use by the care team and documents recommendation made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
- Responsible for outreach efforts to establish and maintain positive working relation ships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff and radiation oncology staff, ).
- Tracking programmatic quality indicators
- Assist patients in understanding their diagnosis, treatment options, and resources available, including educating eligible patients about appropriate clinical research studies and technologies.
- Junior College/Technical School required; Bachelor's degree preferred.
- Current RN license in West Virginia required.
- AHA BLS required.
- ONS certification preferred.
- 2 to 5 Years of Position-Related Experience required - Preference will be given to those experienced in oncology.
