Job Description
  • Coverage Needed:
    • January 1, 2025 - ongoing
    • Call starting Monday at 7am until following Monday at 7am
    • 1 in 3 week call schedule
    • Clinic: Monday-Thursday, 8:30am-4pm
  • Clinical Information:
    • EMR: Epic
    • Inpatient and outpatient if possible, but will take inpatient only
    • Hospital rounding each day
    • Patient volume: busy call schedule, 30 clinic patients day
    • Support Staff: RN, MA and PSR
  • Required:
    • CV must be in Month / Year format
    • Must have license in hand at time of submission, or proof that the license application has been submitted
    • Any gaps greater than 30 days will require an explanation on the CV
    • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
    • Active IN License and DEA (or one that is easily transferrable)
    • Board Certified
    • Must be able to perform emergent procedures
