• Requirements: Board Certified in Family Medicine
• Schedule: Tuesday-Friday, daytime hours

There will be full time clinic coverage. The assignment is ongoing. The provider needs to have an active Wisconsin license and have a clean background to be considered.

• Patient Census: 12-14
