The Family Networks program serves DCF involved families by connecting families being served with needed services. This individual will coordinate services along with DCF to support families based on family needs. Monitor quality of service provided.
The Service Coordinator's Responsibilities are: - Receive case assignments.
- Set up Family Team Meeting (FTM) as required by service type.
- Create service referrals.
- Enter meeting notes into Family Networks dictation.
- Assist with duty day one-two days per week.
- Secure emergency placements when necessary.
- Communicate with Data Coordinator for changes in services via data form/email.
- Communicate ongoing cases with Providers and Social Workers and discussing role of service model-short term/least restrictive/ etc.
- Assist area staff with identifying community resources, brainstorming/problem solving supports for families.
- Attend meetings as needed.
Qualifications: - Bachelor's Degree in Psychology or related field required and two years of direct service experience in child and family services preferred.
- Computer literate in standard desktop applications such as Windows, Word, and Excel
Schedule: Full Time 40 hours - Days
