Bridgeway Inc. is hiring a System of Care Coordinator at our Macomb, IL location to serve Fulton and McDonough counties.The System of Care Coordinator provides primary direction and supervision over the System of Care, as the lead agency, as well as in collaboration with the various communities served and partner organizations. To learn more about this position, please read the summarized list of duties below.Works directly with other community organizations and entities to ensure a "no wrong door approach" for individuals needing assistance in the areas of mental health, substance use, medical, employment, education, housing, food, etc. Acts as the primary coordinator of the community system of care, coordinating meetings, communications, intake/services needed by individuals and data tracking for services provided by the overall system of care providers/stakeholders.Develops appropriate operational policies, procedures, documentation, etc. for the System of Care in coordination with stakeholders.Provides innovative coordinated leadership to assure comprehensive communication, integration of treatment services and coordination with all external resources.Educates and trains Bridgeway staff as well as external organizations and stakeholders in areas related to the System of Care.Ensures all deliverables are met for System of Care funding sources.Maintains program effectiveness, consumer and stakeholder satisfaction and outcome data with a focus on continuous quality improvement.We would love to hear from you! Our minimum requirements include a Bachelor's degree in Behavioral Health, Social Services or helping profession preferred or five years experience in Behavioral Health, Social Services, Healthcare or helping profession will be reviewed.
