Collects patient and family data and reports findings to RN and/or licensed Therapist in a timely manner.Assists in the communication of information to assigned patients.Under the supervision of an RN and/or licensed Therapist, performs interventions and procedures as directed to ensure the safe effective quality of care for patients.Reports to RN and/or licensed Therapist on a variety of aspects of patient and family information including.Advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulation/exercises, etc.Serves as Hospice Aide as directed. Serves as Personal Care Attendant as directed.Proactively reports changes in skin conditions to RN, Case Manager or program manager in a timely manner.
