If you have a passion for working with seniors, the Social Services Designee/Discharge Planner position provides a great opportunity for you The Social Services Designee/Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning. Responsibilities: Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community. Attend the 72 hour care plan meeting. Confirm contact information on face sheet. Ordering or recommending resources to acquire durable medical equipment as needed/recommended by therapy for mobility or nursing to attain optimal health status and ordered by practitioner. Arranging home health care for after discharge with the resident/guest preferred provider, as possible. Ordering and coordinating IV home infusion services, wound vacs, trach supplies, tube feeding supplies, vents and vent supplies, if necessary. Coordinating nursing education with proposed caregiver. Overall coordination of the discharge. Attending care conferences. Attending Medicare Meetings. Any item related to the discharge process for residents who will be going home. Ensuring completion of the Post Discharge Plan of Care and Summary. Education and/or Experience: High school diploma/GED.
