Position Highlights:
Role: PMHNP
Schedule: hours/week (prefer full-time)
Format: Hybrid 2-3 days in the field + telehealth follow-ups & admin tasks
Start Date: ASAP (approximately 1 month for credentialing)
Patient Population: Adults only

Key Responsibilities:
Provide care for Assertive Community Treatment (ACT) patients
Conduct evaluations (45 minutes 1 hour)
Work closely with a supportive team, including an RN, LPN, and a team leader
