The Home Health Aide is a trained individual who provides personal care and related services to patients in their home in order to promote patient dignity and independence as prescribed by the attending physician. The Home Health Aide provides services as directed by the Home Health Case Manager and is responsible to the Clinical and/or Care Manager.
EDUCATION, SPECIAL SKILLS, PHYSICAL ABILITIES REQUIRED : - High School diploma or GED required.
- Successful completion of, and provide documentation of, Certified Nurse's Aide/ Home Health Aide, training totaling at least 75 hours with at least 16 hours devoted to supervised practical training.
- Valid registration with Indiana State Aide Registry Program.
- Successful completion of written and skills competency tests prior to patient care.
- Previous clinical nurse aide experience, one (1) year in hospital, long-term or rehab setting preferred.
- Agrees to have criminal investigation with results complying with IC-16-27-2 Standard.
Valid driver's license and proof of current automobile liability insurance.
