This individual will coordinate services along with DCF to support families based on family needs. Monitor quality of service provided.nnThe Service Coordinator's Responsibilities are:nnReceive case assignments.nnSet up Family Team Meeting (FTM) as required by service type.nnCreate service referrals.nnEnter meeting notes into Family Networks dictation.nnAssist with duty day one-two days per week.nnSecure emergency placements when necessary.nnCommunicate with Data Coordinator for changes in services via data form/email.nnCommunicate ongoing cases with Providers and Social Workers and discussing role of service model-short term/least restrictive/ etc.nnAssist area staff with identifying community resources, brainstorming/problem solving supports for families.nnAttend meetings as needed.nnQualifications:nnBachelor’s Degree in Psychology or related field required and two years of direct service experience in child and family services preferred.nnComputer literate in standard desktop applications such as Windows, Word, and ExcelnnSchedule:nnFull Time 40 hours - DaysnnAs a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
