5,000 sign on bonus ***
Provides case management and direct patient care within a team environment and administers comprehensive physical therapy modalities in the home setting.
Prepares necessary documentation including evaluations, daily notes, progress notes, updated plans of care, discharge summaries and logs in a timely manner according to federal, state and facility-specific rules and regulations.
Consults with other members of the rehabilitation team to select treatment programs consistent with the needs and capabilities of the patient and to coordinate comprehensive services delivery.
Develops effective home programs to maintain and enhance patient/client performance in his/her environment.
Develops and initiate referrals from licensed healthcare professionals. Develops effective rapport with referring healthcare practitioners.
Presents a positive service-oriented approach with patients/clients, family, caregivers, facility staff, and co-workers at all times.
Review the patient/client treatment program to maintain effective and efficient quality service.
Assists with orientation of new staff, volunteers, and students. Supervise physical therapy assistants in accordance with federal and state regulations.
Assists with equipment maintenance and monitor supply inventory.
Demonstrates ongoing commitment to reducing costs and improving productivity in the rehabilitation department.
Maintains positive working relationships with the healthcare team, patient/client, family, and caregiver.
Assists with Quality Assurance reports and records. Complete EveryStep and facility reports as required.
Understands and comply with EveryStep's policies and procedures, including but not limited to, universal precautions, safety, risk, and emergency.
Promotes the EveryStep philosophy, mission, and administrative policies to ensure the delivery of quality care.
Promotes the recognition, respect, and celebration of the diversity of our workforce.
BS, MS, or DPT degree in Physical Therapy
Current and valid Iowa PT license
Current Healthcare Provider CPR certification required.
Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)
Climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, talking, fingering (picking, pinching, typing, etc.), applying pressure with fingers and palm), repetitive motions, feeling (perceiving size, shape, texture, temperature by touching), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)
