Our mission at Apicha Community Health Center is to provide high quality, equitable, whole person and culturally responsive care delivered in an inclusive and welcoming manner. Build and expand on our long tradition of care for AAPI, LGBTQ+ and persons living with HIV. We are currently seeking a Care Coordinator in our 400 Broadway location. Position summary: Lead a team of patient navigators and patient assistant to ensure care coordination services are provided to all clients assigned to meet clients’ needs for better health outcome. Provide day-to-day supervision to the patient navigators and patient assistant assigned by weekly schedule monitoring, chart review, face-to-face case discussion, and performance reviews. Responsible for implementation of service/care plan, supported by Patient Navigators. Conduct outreach & recruitment for Apicha CHC to promote the Agency services. Qualifications: BA/BS in related field with two ( 2) years of qualifying experiences required . An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience may be considered; Experience working with target populations strongly preferred. The target populations: HIV/AIDS who are virally unsuppressed, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis; Ability to adapt changing environment; Appreciation of and commitment to Apicha’s mission and values required; Excellent MS Office skills (Outlook, Word, Excel, PowerPoint) required; Ability to work effectively in both a collaborative as well as individual setting; Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC; Excellent customer service skills to assist in resolving member complaints; Strong verbal and written communication skills – for both internal and external audience.
