CDU was founded to address inadequate medical care in the region and to provide equitable medical education opportunities for underrepresented students.

Job Summary:
Under the general supervision of the PI and Program Manager, provide community outreach, education, and case management services targeting individuals at high risk for HIV and/or individuals living with HIV living in South Los Angeles. Must be knowledgeable of South Los Angeles and proficient in working with populations with HIV or at high risk for HIV, networking in an effort to educate, increase awareness, recruit and enroll eligible individuals into enhanced patient navigation services for PrEP/PEP treatment and medication adherence. Attend care team and community meetings to increase awareness and create referral network among other organizations and groups. Conduct psychosocial assessments and treatment plans and engage/re-engage patients to local clinics for ongoing care. Prior work experience in a social services, health, or substance abuse setting is strongly preferred. This PrEP Navigator will be stationed within one of our partnered county clinics. The schedule will be aligned with their hours of 8am - 4:30pm and their holiday schedule
Essential Duties and Responsibilities:
  • Provide confidential risk and medication adherence assessments to clients in order to assist with risk reduction
  • and adherence.
  • Develops contacts with social service resources available in the area for the clients in need of the services.
  • coordinate services with other social service providers.
  • Collaborates with program staff in advertising and media promotion of the program and assist with recruitment
  • efforts.
  • Engages with community-based organizations and provides education on HIV, STI's, PrEP and PEP in a public
  • forum.
  • Documents program activities, and reports outcomes to the Program Manager.
  • Conduct initial intake assessment and identify appropriate client needs.
  • Conduct programmatic activities by working closely with the program manager, peer support, medical case
  • management, and outreach team members.
  • Provide outreach and follow-up activities; activities may be either on site or off site as needed.
  • Comply with all program protocols and agency policies.
  • Provide and follow up on appropriate referrals to community services.
  • Refer individuals infected with HIV to case management, medical care, support services and other services as
  • needed and thoroughly follow-up on and document all referrals made.
  • Maintain written and computerized records, compile reports and completed other program documentation (e.g.;
  • progress notes, assessments, surveys)
  • Submit clear and accurate data collection forms for all outreach and other work performed.
  • Perform data entry tasks on regular basis.
  • Maintain patient confidentiality and comply with all federal and state health information privacy laws.
  • Deadline driven and detail-oriented, ability to juggle multiple priorities.
  • Attend all required meetings and clinical supervision sessions.

Qualifications/Requirements:
EDUCATION:
  • Bachelor's Degree, or comparable years of qualifying educational/work experience.
  • Must have valid driver's license.
EXPERIENCE:
  • Experience working with at-risk populations and performing outreach.
  • Knowledge and training in HIV/AIDS treatment and prevention are preferred.
KNOWLEDGE/ABILITY/SKILLS :
  • Computer skills including knowledge of database, spreadsheet, and statistical analysis packages are high

  • The ability to perform highly detailed work with sustained attention and care while providing/obtaining information on numerous inquiries is paramount to success in this position.
