TFI Family Services has the job for you We are looking for a full-time Aftercare Case Manager to add to our team All you need is a bachelor's degree in human services and a valid driver's license. We will teach you the rest A Day in The Life as a Case Manager As an Aftercare Case Manager, we provide support and services to children and families that have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. During this work with post-permanency placements the focus is on risk management for their designated aftercare period. This can include crisis intervention for the child/family when necessary, working closely with the public and private agencies providing supportive services, maintaining accurate and timely documentation / reports, coordination of community / educational services. Aftercare Specialists meet with the child/family as determined by the aftercare plan, this is in-person meetings to support the reunification and address or resolve concerns.
