The Nurse Practitioner In-Home Assessment are responsible for performing Health Risk Assessments and occasional transitional care for patients discharged from hospitals to prevent re-admissions. As a Nurse Practitioner In-Home Assessment you will:
- Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
- Educate and counsel patient and family based on assessment results, including recommendations
- Performs follow up visits in transitional care
REQUIREMENTS:
- Current RN and NP licensure in the state of practice
- Current CPR certification
- Must be enrolled in both Medicare & Medicaid
