About the Position:

• Inpatient/Outpatient Hybrid Structure: 80% outpatient and 20% inpatient care.
• Patient Load: Approximately 15-20 outpatient and 5 inpatient patients per day.
• Appointment Structure: Patients scheduled in 20-40-60-minute intervals depending on appointment type for personalized care.
• APP Supervision: Oversight and collaboration with advanced practice providers.
• Embedded Care Coordinator: Enhancing patient care coordination and support within the practice.
