Job Description
Supervises Health Care Aides. Providing guidance, support and mentoring. Evaluating care given according to HCA competencies. Reviewing charting related to client care. Model professional behavior in their communication with HCAs. Fosters positive relationships with HCAs while adhering to professional boundaries. Resolving day-to-day issues brought forward by clients and staff. Referring complex or sensitive issues to the Charge LPN in a timely manner. Review service authorizations\Daily Living Support Plans for appropriateness and completeness. Provide coaching as needed. Escalate performance concerns to the Care Manager as needed Participate in Dofficer program. Report to the Educator on hand hygiene observations. Identify knowledge gaps amongst staff. Review unit processes with LPNs. Facilitating opportunities for staff education. Identify learning needs for HCAs Participate in change management activities • Recruit participants and participate in quality improvement initiatives. Follow up on RLS submissions to identify staff learning needs. Facilitate student learning for HCAs. Participates in the delivery of Health Care Aide or other education modules. Create with the HCA a developmental plan and monitor progress with same. Assist LPN on the floor in response to Emergency situations. Provide direction\support to floor staff as questions arise. May occasionally cover LPN unit shifts if no other alternative reached. Reviewing, recording and completing reports as required. Keep up to date tracking and documentation of education and skills evaluation etc...
As a Licensed Practical Nurse (LPN) Personal Support Coordinator, within the Home Care environment, you are part of an interprofessional health care team responsible for the planning, organizing, implementing, monitoring and evaluating of personal support services. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited Licensed Practical Nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
