These members will include RN-s, psychologists, and Float Nurses. The Behavioral Health Professional is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patent's medical provider and, when appropriate, other mental health providers. The Mental Health manager will be a core member of a synergistic team. This will include not only the community patients provider, but also the medical team.
