ASAP start 30-45 days for credentialing upon receipt of documentation

Initially 90 days to start, option to extend

Must be Board Certified or Eligible

Primarily scheduled patients, 10-15 patients/day on average

Labs, chronic care follow up, xrays, diagnostic review; urgent or emergent patients

Will have a site medical director, 1-2 MD's, & 8 full time NP's

Average rate: $105/hr
