POSITION SUMMARY The Care Management Coordinator provides direct organizational and administrative support to the Care Management Department and the leadership team at Martin Luther King, Jr. Community Hospital. This position will provide employee compliance support to the department staff, management, and Human Resources. This role will be customer service driven and serve as a liaison for the department and collaborates with other members within the organization to provide information and support that are consistent with the goals of the organization and that meets the needs of the executive team staff and vendors we work with. The leadership team will work with this individual to develop process that support continuum of care needs of our patients and prevent discharge delays. ESSENTIAL DUTIES AND RESPONSIBILITIES • Coordinates with administrative assistant to promote professional interaction with other MLK departments and vendors.• Responsible for the upkeep, coordinator, inventory, and tracking of DME program.• Provides assistance with check-in and check-out department resources, not limited to: Care phones, keys, desk, laptops.• Coordinates activities among various departments at Martin Luther King, Jr. Community Hospital for the Care Management leadership team.• Serves as point of contact for submission and collection of department licensure and certifications. Maintains list of renewals and ensures timely submission of certifications. Proactively work with department staff, HR, and department management to ensure process is followed.• Provides assistance and serves as a coordinator to CM Department staff in scheduling of certification courses with FlexEd and/or Education Department course.• Serves as a resource for Employee Health and Education department to ensure compliance requirements are scheduled in a timely manner.• Provide reports to department management in regards to pending items and follow up items assigned by department leadership.• Assist Management team with orientation for new staff.• Assist with invoice processing as directed by management.• Assists Clinical Supervisor and Management with scheduling of meal and rest periods. Provides schedule and reminds staff on days they are present while in unit and reminds staff.• Provides recommendation to improve efficiency, cost savings, outcomes and/or satisfaction related to daily work activities and when collaborating with staff.• Assists with the maintenance of various department files as defined• Coordinates meetings within the office, across the department and in collaboration with our vendors• Assist with incoming calls and/or facilitates telephone calls/messages accurately and in a timely manner• Perform a wide variety of specialized clerical duties involved in the preparation, maintenance, and release of materials related to the department and activities including to organize, process, maintain, update, and route a variety of departmental reports, records, and files• Assists with basic errands and needs of staff to allow them to focus on superior patient care.• Assists with providing creative solutions to recognize and reward staff that perform duties timely and in accordance with Hospital policies.• Adheres to all hospital policies and the Watson Model of Care in all patient/family care and interaction.• Assist with Employee Recognition program and related activities.• Assist with additional projects as assigned.POSITION REQUIREMENTS A. Education• High School Diploma/GED equivalent required. Bachelor's Degree in health related field preferred.B. Qualifications/Experience• One (1) year experience in an administrative/clerical support role required.• One (1) year of Care Management experience required.• Microsoft office intermediate skill level, including outlook scheduling proficiency• Previous experience in business office operations preferred• Invoice process/ authorization/ billing experience preferredC. Special Skills/Knowledge• Excellent customer relations skills required• Strong organizational skills• Ability to work under pressure and to maintain efficiency and composure• Demonstrated knowledge of common microcomputer applications, such as word processing• Typing speed 55+ wpm• Strong interpersonal and communication skills and ability to establish and maintain cooperative and harmonious working relationshipsMLKCH Video
