Job Summary:
DCARA is seeking a Services Coordinator to support our team by managing client referrals, intakes, and documentation, ensuring that all cases are processed and closed efficiently. The Services Coordinator will work closely with department staff and managers to ensure timely and accurate service delivery, enabling clients to access the support they need. This role is crucial in maintaining clear communication and up-to-date documentation in our Salesforce system, helping us maintain client satisfaction and timely follow-up.
Essential Job Functions:
Service Coordination:
- Provide information and referrals to clients via email, videophone, Zoom, and in person.
- Conduct client enrollments and manage an extensive intake process using Salesforce.
- Coordinate service referrals and manage client documentation, including Client Rights & Responsibilities.
- Assign client cases to appropriate staff using Salesforce and ensure timely completion of intake and closure paperwork.
- Administer exit surveys for clients who complete services or relocate out of service areas.
- Collaborate with all DCARA departments to ensure client access to services and accommodations.
Documentation Duties:
- Maintain logs of client information and referrals in Salesforce.
- Manage client file notes and ensure documentation is up-to-date and uploaded in DocuSign.
- Prepare monthly reports and pull statistics from DCARA's database.
Business Relations:
- Process referrals from external service providers, employers, and schools.
- Maintain relationships with external partners to facilitate client referrals.
- Provide information about DCARA services to external stakeholders and ensure a smooth referral process.
Other Job Functions:
- Assist with setting up meeting spaces as needed.
- Greet and assist clients at the office.
- Support the agency and community with other duties as assigned by the Deputy Director.
Required Education and Experience:
- High School Diploma or equivalent.
- Fluency in American Sign Language (ASL) and knowledge of Deaf culture.
- Strong organizational and communication skills.
- Proficiency with Salesforce, Google Suite, Microsoft Office, and DocuSign.
- Ability to work independently, manage multiple tasks, and meet deadlines.
Desired Education and Experience:
- Associate's degree in Social Work, Sociology, or related fields.
- At least one year of experience in social services or working with the Deaf community.
- Knowledge of the Americans with Disabilities Act (ADA) and Section 504.
- Knowledge of Salesforce Database Program
