Under the supervision of the Clinical Manager or designee, the Hospice Care Aide performs patient care activities necessary in caring for the personal needs and comfort of terminally ill patients in various settings.
As a part of the organization, designated hospice services, and the interdisciplinary team, hospice aides have the responsibility to contribute to the comfort and dignity for a patient and provide care that is consistent with patient and family needs and goals; while adhering to the company's mission, vision, and values.
The Hospice Care Aide provides services that are:
• Ordered by the interdisciplinary group.
• Included in the plan of care.
• Permitted to be performed under State law by such Hospice Care Aide.
• Consistent with the Hospice Care Aide training.
Position Details

  • Days/Hours: Monday - Friday, 8:00am-5:00pm
  • Compensation: Base salary $16.00-$17.50/hr; paid driving mileage, plus full benefits package including 23 days annual PTO
  • Position Location: Home health covering Palatka and surrounding areas (Usually West Putnam County, including areas such as Interlachen, Keystone Heights, Melrose, etc...)
Job Responsibilities:
The Hospice Care Aide works along with the interdisciplinary group (IDG) to provide quality patient care. Weekend and holiday work is required. Hospice Care Aides must be willing and able to flex their work location within and between any Community Hospice location or team at any time.
  • Perform hands-on personal care; simple procedures as an extension of therapy or nursing services, usually for 4-6 patients per day
  • Provide assistance in ambulation or exercises.
  • Reports changes in the patient's medical, nursing, rehabilitative, and social needs to a licensed nurse, as the changes relate to the plan of care and quality assessment and improvement activities.
  • Completes appropriate documentation in compliance with the organization's policies and procedures.
  • Teach and train family/caregiver proper techniques to meet the personal care needs of the patient.
  • Attends team meetings and participate in other agency meetings as directed.
  • Maintains a minimum of twelve (12) hours of training/education specific to position annually.
  • Complete mandatory education per company policy.
  • Other duties as assigned.
Education/Experience:
  • High school diploma or GED equivalent preferred.
  • Prefer six (6) months experience.
  • Experience in caring for the terminally ill preferred.
  • Reliable transportation that allows employee to shift work locations when needed
Required Licenses/Certifications:
  • Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida.
  • Current State of Florida Driver's license with driving record acceptable to Community Hospice & Palliative Care insurance carrier; reliable transportation.
  • Current CPR/BLS Certification.
  • Immunizations or screenings as required.
