Position Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team at Harlem Center for Nursing and Rehabilitation. As an LPN, you will play a critical role in providing high-quality care to our residents, supporting their medical and emotional needs, and ensuring their comfort and well-being. You will work closely with our healthcare team to promote health, healing, and rehabilitation.
Key Responsibilities

  • Conduct rounds to observe residents and assess nursing care
  • Review and implement Nursing Care Plans to address residents' physical, emotional, and rehabilitative needs
  • Administer and transcribe physician orders accurately
  • Collaborate with staff to deliver comprehensive, high-quality care
  • Manage medication ordering, administration, and documentation
  • Ensure accurate and thorough documentation of clinical records
  • Participate in interdisciplinary meetings to improve care and resolve issues
  • Provide direct patient care as needed, including monitoring vital signs and administering treatments
  • Assist with identifying and addressing care improvement opportunities
  • Perform additional duties within the LPN scope of practice as necessary
Qualifications
  • Valid New York State LPN license
  • Strong communication, customer service, and interpersonal skills
  • COVID-19 vaccination required
  • Preferred: Experience in long-term care and familiarity with PointClickCare
  • Strong team player with a compassionate, patient-centered approach to care
