We have an opportunity available for a Licensed Practical Nurse (LPN) to provide medical care for skilled nursing residents in a Long-Term Care Facility / Nursing Home in Pittsburgh. As a Licensed Practical Nurse (LPN), you will be responsible for supervising nursing personnel to deliver nursing care and coordinate care delivery, which will ensure that patients' needs are met in accordance with professional standards of practice through physician orders, center policies and procedures, and federal, state, and local guidelines.

Responsibilities:
  • Provide compassionate direct patient care within the scope of the Practical Nurse Act of Pennsylvania.
  • Effectively communicate both verbally and written with all members of the health care team.
  • Assist health care team in the administration of treatment plans and health checks.
  • Administer medications and injections, take vital signs, perform basic diagnostic tests and dress wounds per the health care provider's orders and in accordance with federal, state, local and facility policies and procedures.
  • Observe, evaluate and report changes in patient symptoms, condition, and/or reactions to medications or treatments.
  • Assist and support staff by responding to patient call lights, answer telephones, pass meal trays and transport residents.
  • Observe, evaluate and report changes in patient condition.
  • Communicate with patients and their families to provide a thorough understanding of their treatment plans.
  • Supervise and monitor patient care provided by unlicensed staff.
  • Support patient and staff culture of safety in the workplace.
  • Recognize hazards and report in a timely manner in accordance with facility policies.
Qualifications:
  • Strong clinical skills and ability to react decisively in emergency situations.
  • Impeccable verbal and written communication skills.
  • 1 year of nursing experience is preferred.
The Following Credentials Are Required:
  • Valid Licensed Practical Nurse (LPN) License
  • Valid CPR/BLS Certification
  • Current TB Testing Documentation
  • Current Physical Examination
