Licensed Practical Nurse (LPN) - Developmental Disability & Brain Injury $5,000 Sign On Bonus!***** Description: Eliot is seeking an experienced LPN to coordinate residential services supporting these adults within the communities of Burlington, Haverhill and Framingham. Successful candidates will have 3 years related experience and have skills to establish supportive relationships with individuals with developmental disability and brain injury and commitment to meeting the individual needs and preferences of each person served. Responsibilities: Responsibilities: Provide education and consultation to individuals and staff on medical issues, treatments, medications, wellness and prevention. Provide 1:1 skills trainings in specific individualized need areas and group training for individuals and staff. Provide oversight, guidance and consultation in all residential programs around medical protocols, medication management, and coordination of care. Review all medical documentation, complete medical assessments, and prepare medical documentation for all new referrals and discharges. Participate in transition planning and coordination of medical and nursing care/services as needed for new referrals and current case load. Perform direct nursing care and medical support services to all residential programs as needed. Hire train and supervise nursing staff for the Brain Injury programs. Provide regular supervision and guidance to include job performance evaluation and disciplinary action, as needed. Meet with managers and nursing staff when an individual receives any new medical diagnosis, is prescribed a new medication, diet, medical device or protocol to provide training, consultation and protocols for on-going assessment of symptoms, treatment effectiveness and monitoring of appropriate responses/actions. Participate in the development of treatment goals for each individual related to medical, medication or health concerns. Ensure that each individual is actively involved in their treatment planning process as much as they are able to be. Develop working relationships with community medical providers and VNA related services in order to assist with coordinating medical care for individuals. Obtain and maintain MAP Trainer certification and conduct MAP certification and MAP Refresher training for staff on an ongoing, scheduled basis. Ensure that all programs are in compliance with DPH/MAP regulations. Review scheduled medication audits submitted by managers and follow up on all deficiencies. Conduct regular MAP audits in all programs and unscheduled audits as assigned by supervisor. Ensure program MCSR certificates remain current and apply for renewals before expiration. Schedule: Full-time, M-F, 8am-4pm or 9am-5pm Salary: $62,680.00 Qualifications Skills Preferred Communication Skills-Expert Computer Software Skills-Expert Med Administration-Expert Education Preferred Bachelors or better in Nursing.
