You will have the opportunity to collaborate with our healthcare team to implement individualized treatment plans for each assigned resident, ensuring their unique needs are satisfied. Additionally, you will supervise and evaluate CNAs, ensuring that they are delivering care following our high standards. Your responsibilities will also include overseeing our fall-risk and pressure ulcer prevention programs, and actively working to prevent and manage these conditions. Furthermore, you will conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary.nnREQUIREMENTSnnActive license to practice as an LPNnnExperience in long-term care, skilled nursing or rehabilitation facilities is preferred.nnLEARN ABOUT USnnWe are a leading provider of senior healthcare services specializing in post-acute care.
