Position Summary: CCI Health Services (CCI) works with the Maryland Department of Health (MDH) to provide comprehensive health assessments, appropriate follow-up care and referrals to newly arrived refugees, asylees, those with special immigrant visas (SIVs), victims of trafficking, and humanitarian parolees resettling in Montgomery and Prince George's Counties.
The Registered Nurse (RN) within the Refugee Health Assessment (RHA) program, under the direction of the Director of Nursing, is a member of the CCI nursing team, and is responsible for assisting and supporting the RHA program and the CCI clinical team in providing high quality, direct patient care assessments, coordination, and education. The RN assists with planning and implementing work processes and ensures all core activities delineated in the agreement with MDH are performed and follow relevant (MDH< CDC) guidelines.
KEY FUNCTIONS AND RESPONSIBILITIES : - Serves as Registered Nurse for the Refugee Health Program and for primary care at all health center locations as needed.
- Performs routine nursing care and care management duties, upholding high quality and safety standards for the Refugee Health Program and primary care at all health center locations, which may include:
- Collecting patient health history, assessing, screening, analyzing and identifying a patient's healthcare needs including screening for conditions such as diabetes, hypertension, tuberculosis, STDs and parasitic disease.
- Reviewing and reconciling international medical and vaccine records.
- Screening for behavioral health needs and referring to appropriate services.
- Reconciling medication history and counseling patients on the use, benefits, risks, and potential side and adverse effects of any medications prescribed.
- Determining a patient's care gaps and developing plans of care to address them.
- Counseling and education on abnormal labs and working with care teams to determine appropriate follow-up screening and notification to local and state agencies as indicated.
- Provides verbal and written information on identified health and health literacy needs, in the patient's preferred language by utilizing in-person or telephonic interpreters.
- Counseling on social determinants of health and acclimating to their new environment including, for example, how to obtain health insurance, where to buy groceries, how to use public transportation and where to fill prescriptions.
- Provides and oversees clinical coordination with the patient and the referring agencies, including referring to other services within the CCI system such as prenatal care, dental care, and HIV care.
- Plans, coordinates, and/or attends assigned meetings, health fairs, events, and trainings.
- Assists in the management, documentation, and development of the Refugee Health Programs and nursing department programs.
- Assists with training new employees and ongoing training of medical assistants and other support staff.
- May directly supervise medical assistants per the direction of the nursing and program leadership and assist with managing the team's schedule.
- Uses the Electronic Medical Record to maintain patient medical records, utilizing standard language and abbreviations and following accepted medical standards for charting.
- Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive, and trauma-informed manner.
- Works within the policies and procedures of CCI Health Services.
Specific Job Skills: - Ability to implement, monitor, and manage grant-specific requirements within the primary care setting.
- Professional nursing skills are required and include but are not limited to the following:
- Critical thinking and assessment.
- Problem analysis and proposal of resolutions based upon available resources.
- Multidisciplinary team collaboration, strong communication skills, and leadership abilities.
Minimum Qualifications: - Bachelor of Science in Nursing.
- Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.
- Current BLS/CPR certification required.
- Experience with documenting in an Electronic Medical Record (EMR) - specific knowledge of eClinicalWorks is a plus.
- A minimum of 2 years of nursing experience.
- Written and oral fluency in English.
- Experience with management of grant-funded programs a plus.
- Experience in a culturally diverse community healthcare setting.
