Rate Caps for Submission:
Malpractice - Must be included in rates.
Orientation - half the hourly/callback rate*** kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"
Hourly - under *** - "Per hour, not OT"
Holiday - Must be 1.5x the regular rates "Per holiday hour, no OT"
Nightly Call - under *** - "Per night call to hold the pager, to include 2.5 hours gratis"
Holiday Nightly Call - "Per night call to hold the pager, to include 2.5 hours gratis"
On Call (Daily) - under *** - "Per 24 hours to include 4 hours gratis"
24 Hour Holiday Call Rate - "Per *** hours to include 4 hours gratis"
Call Back -under *** - "Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."
Holiday Call Back - "Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."
Schedule/Availability Requirements:
In person orientation Jan 30-31 if provider is picking up Feb ongoing dates. If provider picks up January dates we will most likely need to do an off cycle orientation.

Facility Location: Virginia Mason Medical Center
Setting: Hospital
Duties: responsible for referral to Interventional Radiology. Discuss cases with referring physicians and provide prompt service for the hospital and outpatient cases.
Required Procedures: Drain placement, Biliary interventions, Urologic interventions, Hepatobiliary interventions including TIPS, BRTO, GI bleed embolization. PE thrombolysis or thrombectomy,

Board Certified - Highly preferred. Will consider candidates BC/BE in Radiology with IR experience!
State License - Highly preferred but will consider IMLC LOQ candidates
DEA and BLS - Required during credentialing
Fluoroscopy certificate - Required, but can be flexible on this requirement.
