Clinician will promote delivery of quality care and efficient utilization of agency resources and interdisciplinary collaboration to establish a plan of care. Support of adult patients in the community with a concentration on the adult cardiac/ heart failure population ranging from moderate to complex illness in the home. Focus on delivering efficient, cost-effective health care services by way of optimization of patient health and reduction in hospitalization. Clinician will have the ability to rapidly identify changes in patient condition and assess knowledge gaps, ultimately fostering successful independent community-based disease management. Position Responsibilities:nnConducts comprehensive patient assessments incorporating psychosocial and physiological status as well as health behaviors, compliance with therapeutic regime, interventions and technical procedures.nnInterdisciplinary collaboration through extensive communication and collaboration with other members of the healthcare team, patients, patient families and caregivers to develop the plan of care.nnCoordinates plan of treatment with the physician through timely and appropriate communication.nnDemonstrates complete knowledge of established standards for documentation, use of point of service device, and appropriate time frames for completion.nnMaintains purposeful use of resources including home visit supplies, patient disease management tools and time management for quality and efficiency in delivery of care.nnEducates patient and family / caregiver regarding disease process, individual care needs, wellness, medications, safety issues, and long-term management.nnMaintains standard precautions, professional and departmental competencies, and patient safety principles.nnSupervises the Home Health Aide in the home.nnIdentifies and participates in performance improvement activities.nnPosition Qualifications Required / Experience Required:Must have a valid NJ RN License. Approved licenses must be presented and maintained in accordance with the State of New Jersey Licensing Board.nnMust have a current BLS CPR certification.nnMust have valid NJ driver’s license, auto insurance and personal vehicle. 1 - 2 years recent work experience in a hospital i.e. Med/Surg, E.R., OB/GYN, Peds, MICU, CCU or other specialty units and/or one-year recent community health experience. Direct care and management of cardiac and/or heart failure specific patients.Broad knowledge of nursing practice and ability to apply specific skills in the home health setting.nnPreferred:nnOne-year recent community/home health experience.nnCardiac Certification.Required Education:Baccalaureate Degree preferred.
