Job Responsibilities
With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.
This position is located at the Department of Mental Health, Charleston- Dorchester Mental Health Center, 2100 Charlie Hall Blvd, Charleston, SC 29414.
Assertive Community Treatment (ACT) Team Registered Nurse:
This position is for a Registered Nurse (RN) for our new Assertive Community Treatment (ACT) team. This multidisciplinary program utilizes a team-oriented approach to providing care for patients who need higher levels of care with intervention and treatment and supervision in the home and community. The intended outcomes of this program are increasing community living skills, decreasing psychiatric hospitalizations, and decreasing criminal justice involvement. Assertive engagement and outreach techniques are used to engage patients in services.
The RN will be part of this multidisciplinary team, providing direct nursing services as credentialed to ACT patients, including but not limited to nursing assessment, crisis intervention, medication management and monitoring, medication administration and injections. Services are provided in home and community settings as well as in the clinic and via telehealth when appropriate. We seek compassionate individuals who are driven to provide outstanding care to those we serve.
Staff members are responsible for documenting services in a timely manner using an Electronic Medical Record system. All necessary technology will be provided.

Minimum and Additional Requirements
State Minimum Requirements:
Graduation from an accredited school of nursing.
Agency (Facility) Minimum Requirements:
A bachelor's degree in nursing and two (2) years experience; or a three (3) year diploma and two (2) years experience; or an associate degree in nursing and two (2) years experience as a registered nurse, one (1) of which must have been in a community health program. One year experience working with adults diagnosed with serious mental illness and working knowledge of psychiatric medications. Licensed as a registered nurse by the SC Board of Nursing.

Preferred Qualifications
Knowledge of the techniques and best practices of the nursing professional.
Knowledge of medical and nursing terminology. Ability to evaluate a client's medical status and needs and use knowledge and judgment to respond appropriately. Ability to plan, implement and evaluate client medical care plans. Ability to provide individual health education and counseling. Ability to communicate effectively both orally and in writing. Ability to function as a team member. Knowledge of privacy laws.
