Reporting to the Clinical / Center Service Director, the RN Care Delivery Field Supervisor will oversee and ensure high-quality clinical services for patients in their homes. This role is integral to maintaining our nursing standards of excellence and delivering care that is sensitive to diverse backgrounds and needs. Make a difference in the lives of the geriatric population by ensuring they receive high-quality home health care.nnKey Responsibilities:nnSupervision & Delegation: Collaborate with the Care Delivery Manager to supervise and delegate responsibilities to assigned personnel, ensuring quality care and adherence to care plans.nnAssessment & Evaluation: Assess and evaluate participants' health care needs, considering physical, mental, and environmental factors.nnGuidance & Instruction: Supervise and guide Community Health Nurses (CHNs) and Home Health Aides (HHAs) in delivering quality home health care services.nnJoint Visits & Competencies: Conduct joint visits with CHNs for supervision and competency assessment.nnOrientation & Training: Assist in designing orientation programs for new employees and provide education to participants and aides on medications, safety, and diet.nnCollaboration & Care Planning: Work with the interdisciplinary team to develop comprehensive care plans tailored to individual participant needs.nnDocumentation & Reporting: Maintain accurate documentation and submit reports as required by Health System standards and federal/state regulations.nnField Visits & Audits: Conduct field visits in place of CHNs as needed and perform routine chart audits.nnTravel: Travel is required, including potential coverage of neighboring sites.nnRegulatory Compliance: Act as a core interdisciplinary team member, ensuring compliance with CMS PACE regulatory requirements.nnQualifications:nnEducation:nnDegree from an NLN-accredited nursing school (RN, BSN required).nnBaccalaureate degree highly desired.nnAt least five (5) years of experience in a healthcare setting is acceptable in lieu of BSN.nnExperience:nnMinimum of one (1) year of experience as a community health nurse in a Certified Home Health Agency, Long Term Home Health Care Program, or Licensed Agency.nnExperience working with geriatric populations preferred.nnExperience in a PACE Program or MLTC preferred.nnManaged long-term care insurance experience beneficial.nnSkills:nnExcellent organizational and presentation skills.nnProficient with computers and laptops.nnStrong knowledge of New York State Department of Health regulations.nnLicensure:nnCurrent active and unrestricted license and registration in New York State as a Registered Nurse required.nnValid New York State driver's license and reliable transportation preferred.nn*Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
