The Hospice RN Team Lead is responsible for providing leadership and direction to the designated team of nurses to ensure the highest standards of patient care.nnThe Team Lead is responsible for developing and implementing patient care plans, assessing patient needs, and ensuring that all care is delivered in accordance with established protocols and best practices.nnResponsibilities:nnParticipates in the coordination and planning of the delivery of quality, cost-effective hospice care.nnEvaluates patient services provided by the interdisciplinary team.nnMaintains working knowledge of hospice state licensure, federal and accreditation requirements.nnMonitor services and documentation to meet hospice policies and procedures, state, federal, and accreditation guidelines.nnServe as a liaison to the Administrator regarding clinical challenges and progress in patient care.nnSupervise and participate in the provision of care provided, orientation, scheduling, education, and support of all caregivers on the hospice interdisciplinary team, including any contracted staff.nnBe available on-call if needed.
